INFORMATION CLAUSE RELATING TO THE PROCESSING OF PERSONAL DATA

​Pursuant to Art. 13 sec. 1 and sec. 2 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46 / EC (hereinafter: "GDPR "), I would like to inform you that:

I. Personal data controller.

III. Type of personal data processed.
The processing of personal data includes the following types of data:
- name (s) and surname,
- company name;
- e-mail adress,
- Phone number,
- correspondence address,
- data provided by you during the performance of the contract concluded with the Administrator in connection with the legal services provided,
- data obtained from other persons during the performance of the contract (e.g. the opposing party, court, bailiff and others).
IV. The purpose of processing personal data and the legal basis for their processing.
1. Your personal data will be processed in accordance with the principles set out in Art. 5 GDPR and art. 6 of the Act of May 26, 1982. The Law on the Bar (consolidated text Journal of Laws 2017, item 2368, as amended) in order to:
a.the Administrator's compliance with the obligations arising from the contract for the provision of legal assistance, in accordance with art. 6 sec. 1 lit. a GDPR,
b. to fulfill the Administrator's obligations in the interests of the data subject prior to concluding the contract, in accordance with art. 6 sec. 1 lit. b GDPR,
c. fulfill the legal obligation incumbent on the Administrator, in accordance with art. 6 sec. 1 lit. c GDPR.
2. Providing personal data is voluntary. 
V. Recipients of personal data.
1. Your personal data may be shared with the following entities:
- entities dealing with accounting and accounting services as part of the Administrator's business activity;
- entities involved in the operation of IT systems as part of the Administrator's business activity;
- common courts, administrative courts, administrative bodies;
- entities cooperating with the Administrator for the purposes of providing legal assistance services or for the purpose of fulfilling obligations under the law.
-Mr / Mr * personal data may be transferred only to lawyers whose provisions impose the obligation of secrecy - only to enable the replacement of the attorney in charge of your case * or the employees of the law firm who have been entrusted with the processing of personal data in writing and who are responsible for the breach of the rules processing.
2. The administrator does not intend to transfer your personal data to a third country or to international organizations.
VI. The period of processing and storage of personal data.
Your personal data will be processed until the purpose of their processing in connection with the concluded contract for the provision of legal assistance ceases or until consent to their processing is withdrawn, but no longer than until the claim is time-barred or the conviction is expunged.
VII. Rights of persons whose personal data are processed.
The data subject has the following rights:
a.right to access its data, in accordance with art. 24 sec. 1 point 3 UODO and art. 15 GDPR;
b. the right to rectify its data, in accordance with art. 24 sec. 1 point 3 UODO and art. 16 GDPR;
c. the right to delete their data ("the right to be forgotten"), in accordance with art. 17 GDPR;
d. the right to limit its data, in accordance with art. 18 GDPR;
e. the right to transfer its data, in accordance with art. 20 GDPR;
f. the right to object to data processing, pursuant to art. 21 GDPR.
You * have the right to withdraw your * consent to the processing of your * personal data (including those belonging to a special category) at any time, but the withdrawal of consent does not affect the lawfulness of the processing that was carried out in accordance with the law, before its withdrawal .
In order to exercise the above-mentioned rights, please contact the Administrator in one of the ways indicated in point II of this information clause.
The data subject also has the right to lodge a complaint with the supervisory body - the President of the Personal Data Protection Office, if he or she believes that the processing of his personal data violates the GDPR.
